Background:
In 2023, the Delray Beach Community Redevelopment Agency (CRA) began constructing a new, approximately 7,089 square foot, two-story commercial building located at 95 SW 5th Avenue. The ground floor spans approximately 2,245 square feet, and will feature a lobby, staircase, restrooms, elevator, and one (1) commercial rental space accessible from SW 5th Avenue. The second-floor spans approximately 4,844 square feet and will feature restrooms and two (2) commercial rental spaces. The development project signifies an important step toward creating affordable commercial rental space and activation along the Historic Northwest & Southwest 5th Avenue commercial corridor, as called for by the CRA Redevelopment Plan and The Set Transformation Plan.
In 2024, the CRA Board prioritized meeting the needs of The Set community as identified in The Set Transformation Plan, and the CRA’s Redevelopment Plan, and dedicated the affordable commercial rental spaces within 95 SW 5th Avenue for tenants who would provide accessible and affordable medical, pharmacy, and healthcare services. Construction of the commercial building is complete and awaiting final inspection, with the three (3) tenant suites awaiting final buildout based on the needs of the tenant.
Notice of Intent to Lease:
On December 10, 2025, pursuant to Florida Statute 163.380, a Notice of Intent to Lease the three (3) tenant suites within 95 SW 5th Avenue was issued with an initial Submission Due Date of January 30, 2026. The Submission Due Date was extended to March 31, 2026.
At the time of the March 31, 2026, Submission Due Date, three (3) applications were received. CRA Staff reviewed the applications and determined that the applicants and applications would not provide the accessible and affordable medical, pharmacy, and healthcare services to meet the needs of The Set community.
On May 20, 2026, a second Notice of Intent to Lease was issued with a Submission Due Date of July 6, 2026.
Below is a summary of the Notice of Intent to Lease:
Overview of Tenant Suites and Lease Information:
• One Ground Floor Suite - Approximately 1,075 square feet
• Two Second Floor Suites - Approximately 1,228 and 2,504 square feet
• Lobby with interior elevator
• Access to on-site parking and public parking lot nearby
• Approximate monthly rental rate (not including utilities, internet, phone, security, interior pest control):
o Rental Rate to be negotiated with each tenant
o Negotiated Rental Rate will increase 6% annually
o Rental Term: Ten (10) years with options to renew
• All three (3) suites are ready for tenant build- out and may all be leased by one (1)tenant or may be leased to three (3) separate tenants.
• Eligibility to apply for CRA Funding Assistance Programs for interior renovations
Applicant Criteria:
• Applicant must be a medical, healthcare, or pharmacy service provider that offers affordable medical, healthcare, or pharmacy services that may be used by residents of The Set.
• Applicant must accept a range of health insurance types, including Medicare and Medicaid.
• Applicant must hold an active State of Florida license necessary to provide the proposed medical, healthcare, or pharmacy services at the Rental Space.
• Applicant must demonstrate that they have a minimum of two (2) years' of experience as of the due date of this Application managing and operating a medical, healthcare, or pharmacy office that accepts a range of health insurance types, or two (2) years' of equivalent, relevant experience providing medical, healthcare, or pharmacy services, and performing the equivalent, relevant duties and responsibilities required to operate a medical, healthcare, or pharmacy office that accepts a range of health insurance types.
Tenant Application Received:
The only application received by the July 6, 2026, Submission Due date was from Florida Atlantic University Community (FAU) Health/Northwest Community Health Alliance (NCHA).
The FAU/NCHA operates nurse-led community health centers that are Federally Qualified Health Center (FQHC) Look-Alikes. The community health centers are operated through a partnership between FAU’s Christine E. Lynn College of Nursing and NCHA that was created in 2014 to address healthcare access gaps by providing community-based, integrated primary care and behavioral health services. The FAU/NCHA Community Health Centers are dedicated to providing comprehensive, affordable, and accessible healthcare services to underserved and vulnerable populations throughout Palm Beach County.
FAU/NCHA Community Health Centers are currently located in Westgate and at the U.B. Kinsey Educational and Community Center in West Palm Beach. Both locations serve children, adolescents, adults, and older adults.
At its current locations, the FAU/NCHA Community Health Centers provide comprehensive primary care and behavioral health services, including routine medical care, chronic disease management for conditions such as diabetes, hypertension, and asthma, treatment for acute illnesses and urgent healthcare needs, psychiatric evaluation and medication management, preventive health screenings, wellness education, care coordination, laboratory services, and community resource referrals. The FAU/NCHA Community Health Centers accept Medicare, Medicaid, commercial insurance, and also provide access for qualifying uninsured individuals through available programs and sliding fee opportunities.
At the CRA-owned building located at 95 SW 5th Avenue, the FAU/NCHA Community Health Center plans to relocate from its West Palm Beach location and expand its access to integrated community-based healthcare services for residents in Delray Beach, and the greater surrounding area covering Boynton Beach and Boca Raton. The focus of the FAU/NCHA Community Health Center is to improve access to preventive, primary, and behavioral healthcare close to where patients live, work, and attend school.
At this new Delray Beach location, the FAU/NCHA plans to continue providing comprehensive primary care services for children, adolescents, adults, and older adults, including wellness visits, preventive care, chronic disease management, acute illness treatment, and routine medical care. FAU/NCHA will also expand its behavioral health and mental health services, including psychiatric evaluation and medication management, while also offering health screenings, wellness education, care coordination, laboratory support, and community resource referrals. In addition, the Delray Beach location is intended to enhance access to pediatric care, increase exam room capacity, create space for health education and community programming, and support the future inclusion of on-site pharmacy services to further improve medication access and continuity of care for the surrounding community.
The Delray Beach location of the FAU/NCHA Community Health Center will continue to serve a diverse patient population across southern Palm Beach County, with a primary focus on underserved, vulnerable, and medically at-risk individuals and families in Delray Beach, and the greater surrounding area covering Boynton Beach and Boca Raton. Services and care will be provided to children, adolescents, adults, and older adults, including patients with chronic medical conditions, behavioral health needs, and those who may face barriers to accessing traditional healthcare services due to transportation, cost, insurance limitations, or healthcare provider shortages. The Delray Beach location of the FAU/NCHA Community Health Center is intended to improve access to affordable, integrated primary and behavioral healthcare for Medicare, Medicaid, commercially insured, and qualifying uninsured patients, while expanding services for families, seniors, and community members in need of convenient, community-based care.
Board Action:
At this time, CRA Staff is requesting the CRA Board award the Notice of Intent to Lease the three (3) tenant suites within the CRA-owned commercial building located at 95 SW 5th Avenue to Florida Atlantic University Community Health/Northwest Community Health Alliance to operate a community health center and authorize CRA Staff to enter into lease negotiations with Florida Atlantic University Community Health/Northwest Community Health Alliance. The negotiated lease agreement will be brought before the CRA Board for approval.
