Motion to approve Resolution No. 15-19 to award an agreement with Life Extension Clinics, Inc. d/b/a Life Scan Wellness Centers for health screening physicals utilizing City of Jacksonville Solicitation No. P-05-18.

Background:
On March 2, 2018, City of Jacksonville (Jacksonville) entered into a one-year agreement with Life Extension Clinics, Inc. d/b/a Life Scan Wellness Centers (Life Scan Wellness Centers) for health screening physicals (Jacksonville Contract). The Jacksonville Contract is valid through March 1, 2019, with the option to renew for up to an additional three one-year options.
The City of Delray Beach (City) has an ongoing need for health screening physicals offered by Life Scan Wellness Centers through the Jacksonville Contract. Such services are required to check the City’s Fire-Rescue personnel for cancer due to their work environment, as well as remain compliant with Collective Bargaining Agreements. Jacksonville anticipates renewing their contract for the first one-year renewal option. At that time, the City’s Agreement will automatically renew, unless otherwise terminated.
Delray Beach Fire-Rescue budgets $62,700 per fiscal year for the health screening physicals, thus making the total Agreement value $198,550 for a potential contract term of three years and two months (January 15, 2019 through March 1, 2022).
This motion is in accordance with the Code of Ordinances Chapter 36, Commission Approval Required and Purchasing Policies, Accessing Contracts from Other Government Agencies and Not-for-Profit Organizations.
