Motion to approve an increase to the contract value in the estimated amount of $139,555 through March 1, 2022, with Life Extension Clinics, Inc. d/b/a Life Scan Wellness Centers for the purchase of health screening physicals.

Background:
On March 2, 2018, the City of Jacksonville (Jacksonville) entered into a one-year agreement with Life Extension Clinics, Inc. d/b/a Life Scan Wellness Centers (Life Scan Wellness Centers) for health screening physicals (Jacksonville Contract). The initial term of the Jacksonville Contract was valid through March 1, 2019, with the option to renew for up to an additional three (3), one-year options. Jacksonville subsequently exercised the renewal options. The agreement will expire on March 1, 2022.

On January 15, 2019, the City approved resolution 15-19 to award an agreement with Life Scan Wellness Centers for health screening physicals utilizing the City of Jacksonville contract.

The Fire-Rescue Department and the Police Department currently use this Agreement and are requesting an increase in spending authority in the total amount of $139,555. The potential usage of the Agreement by the Police Department and the increase in the number of Fire-Rescue health screening physicals by the Fire Rescue Department were not contemplated when the agreement was originally taken to City Commission for approval in 2019.
